Current membership options
The legacy membership page states that Planrise no longer administers the discount plan. Existing members keep benefits until expiration; renewals and new sign-ups are handled by Centre Dental staff.

Each member must keep a valid credit card on file for auto-renewal. Lapsed membership may require a reactivation fee. Membership is for uninsured patients, is not transferable, and unused benefits expire when the membership expires. No refund is allowed once any membership service item is used.
